The Youth Justice Victims Register can provide eligible victims with information about a young offender who has received a custodial sentence for a criminal act of violence, or offences within the context of family violence. 

The young offender must be serving a sentence in a youth justice custodial centre or on parole. 

The Youth Justice Victims Register cannot provide information about young offenders on remand (in custody awaiting sentencing) or young offenders on bail (in the community awaiting sentencing). 

The Youth Justice Victims Register is not a financial assistance service.

Who should complete this form

Please complete this form if you would prefer another person (nominee) receive information from the Youth Justice Victims Register on your behalf. Your nominee should be someone you trust and remain in regular contact with, for example: a close friend, family member, or case worker.

This application form can be completed online, if both you and your nominee have email addresses and it is safe for the Youth Justice Victims Register to send emails.

Who can apply to go on the Youth Justice Victims Register

You are eligible for the Youth Justice Victims Register if a criminal act of violence was committed against you on or after 30 September 2026, and you are:

  • a primary victim who was harmed by the offence

  • the next of kin or primary carer of a person who died as a result of the offence

  • the primary carer of:

    • a victim who is under 18 years of age

    • a victim who is incapable of managing their own affairs.

You are also eligible if you are a victim with a documented history of family violence committed by the young offender.

If you apply to go on the register as a victim of family violence, the young offender’s sentence in a youth justice custodial centre does not need to be for a criminal act of violence or an offence against you.

What information is required for my application

You will be asked to provide supporting documentation for your application, including:

  • identification document

  • relationship document (if you are applying as next of kin or primary carer)

  • information about the young offender and the offence (e.g. name of the young offender or police incident number).

Primary identification documents

  • Birth certificate

  • Valid passport

  • Valid Australian driver licence

  • Proof of age card (issued by the Australian Government)

  • Immicard (issued by the Department of Home Affairs)

  • Valid Australian student card (for applicants under the age of 18 years)

If a primary identification document is not available, you can provide one Category A document AND one Category B document from the below list:

Secondary identification documents - Category A

  • Expired Australian driver licence (up to 3 months expired)

  • Expired Australian passport (up to 3 months expired)

  • An overseas passport (if expired by more than two years, it is acceptable if accompanied by a current Australian visa)

  • Certificate of identity issued by the Passport Office

  • Valid Victorian marine licence 

Secondary identification documents - Category B

  • Medicare card

  • Department of Veterans Affairs card

  • Pensioner concession card

  • Current entitlement card issued by the Commonwealth (Centrelink/Health Care card)

  • Any Australian or overseas credit card or account card from a bank, building society, or credit union

  • Working with Children check card

  • Australian Keypass card

Relationship documents (if you are applying as next of kin or primary carer)

  • Birth certificate

  • Death certificate

  • Guardianship order

  • Marriage certificate

  • Power of attorney

  • A witnessed statutory declaration detailing your relationship with the victim, AND one of the following:

    • Medicare card listing both applicant and primary victim’s names

    • Health care card listing both names

    • Official correspondence from the Office of Public Prosecutions or Victoria Police confirming the relationship

What happens after I complete my application?

Once you have completed all of your details for the application, an email will be sent to your nominee requesting they complete their section of the application.

When your nominee has completed their section of the application, an email will be sent to you requesting you complete your declaration, then your application will be submitted to the Youth Justice Victims Register.

The Youth Justice Victims Register will contact you in approximately 7 to 14 days once your application has been processed.

Why we collect your information

We collect the personal information you provide in this application because it is required under the Youth Justice Act 2024 to: 

  • assess your eligibility for inclusion on the Youth Justice Victims Register 

  • provide information and notifications to you about the relevant young offender 

  • make sure that disclosure of the young offender’s information is appropriate by considering risk of harm to you and any potential risk of harm to the young offender in all the circumstances.

We may also collect additional information from Victoria Police, the Melbourne Children’s Courts or Melbourne Magistrates' Courts to assess your application.  

Who we share your information with

Offenders are not informed when a person is placed on the Youth Justice Victims Register. Your name and personal details will never be disclosed to an offender. 

We may share your information within DJCS where necessary to assess your application and administer victim services. This may include the Victims Register if you provide details of an adult offender in your application. 

We may also share your information, or confirm the information you provide, with: 

  • Victoria Police or other law enforcement agencies as defined in the Privacy and Data Protection Act 2014 

  • Victorian Courts 

  • the Youth Parole Board 

  • Youth Justice 

This is so we can confirm your eligibility to be on the Youth Justice Victims Register, and so that agencies can fulfil their obligations to you as a registered victim or assess matters relating to the relevant young offender.  We may also seek further information about you from Youth Justice so that we can consider if disclosure of discretionary information is appropriate considering any risk of harm to you and the relevant young offender.

We may also share information with the above organisations, or seek further information from those agencies, about any nominee that you seek to appoint.  

What happens if you do not provide the information?

Applying to be included on the Youth Justice Victims Register is voluntary. 

However, if you do not provide the required information or supporting documents: 

  • we may not be able to assess your application, and 

  • you may not be able to be included on the Youth Justice Victims Register. 

Some questions in this form are optional. Choosing “prefer not to respond” to optional questions will not affect your application.

Eligibility Questions

Young offenders are typically aged between 12-18 years. The term 'young offender' can also refer to a young adult up to the age of 24 years, who is serving their sentence in a youth justice custodial centre instead of an adult prison. Young offenders are usually dealt with in the Children’s Court, but may be dealt with in adult Courts. In Victoria, the law says that a child aged under 12 years old cannot be charged with a criminal offence.

Serving a sentence means the young offender has been sentenced by a Court to serve a period of time in custody in a youth justice custodial centre (secure facility for young offenders who have been remanded or sentenced after committing a crime). 

Parole refers to the young offender serving part of their custodial sentence under supervision in the community.

Family violence is a pattern of behaviour where one person tries to control or scare another person. It is a form of abuse that can happen in many different types of relationships. Examples of a documented history of family violence include: an intervention order (current or expired), police report, or evidence of reporting the family violence to a relevant professional.

A criminal act of violence includes assault, armed robbery, home invasion, carjacking, stalking, kidnapping, family violence, threats to kill, sexual offences, culpable driving, manslaughter and murder (this is not an exhaustive list).

The Youth Justice Victims Register can only provide information about a young offender who is currently serving a sentence in a youth justice custodial centre or is on parole in Victoria, where the criminal act of violence was committed on or after 30 September 2026.

Consent to be Contacted by the Victims Register

The Youth Justice Victims Register is only for victims of young offenders who are serving a sentence in a youth justice custodial centre or on parole. The adult Victims Register is available for victims of adult offenders who are serving a prison sentence or on parole.

If the young offender is in an adult prison or transferred to an adult prison and you tick yes, the adult Victims Register will contact you to discuss registering on the adult Victims Register so that you continue to receive updates.

Identification Documents

All applicants must provide a valid form of identification. 

Primary identification documents

  • Birth certificate

  • Valid passport

  • Valid Australian driver licence

  • Proof of age card (issued by the Australian Government)

  • Immicard (issued by the Department of Home Affairs)

  • Valid Australian student card (for applicants under the age of 18 years)

If a primary identification document is not available, you can provide one Category A document AND one Category B document.

Secondary identification documents - Category A

  • Expired Australian driver licence (up to 3 months expired)

  • Expired Australian passport (up to 3 months expired)

  • An overseas passport (if expired by more than two years, it is acceptable if accompanied by a current Australian visa)

  • Certificate of identity issued by the Passport Office

  • Valid Victorian marine licence 

Secondary identification documents - Category B

  • Medicare card

  • Department of Veterans Affairs card

  • Pensioner concession card

  • Current entitlement card issued by the Commonwealth (Centrelink/Health Care card)

  • Any Australian or overseas credit card or account card from a bank, building society, or credit union

  • Working with Children check card

  • Australian Keypass card

Category B - Relationship documents

If you are applying under Category B (Next of kin)

Please include one document that demonstrates your relationship to the victim, for example:

  • birth certificate

  • death certificate

  • guardianship order

  • marriage certificate

  • power of attorney

This document helps ensure that private and confidential information about the crime the victim experienced is only shared with their next of kin.

If one of the above documents is not available, you can provide:

  • A witnessed statutory declaration detailing your relationship with the victim, AND one of the following:

    • Medicare card listing both applicant and primary victim’s names

    • Health care card listing both names

    • Official correspondence from the Office of Public Prosecutions or Victoria Police confirming the relationship

Category D - Supporting Documents

If you are applying under Category D, please include documentation related to the history of family violence, such as a family violence intervention order. If you provide an expired family violence intervention order, the Youth Justice Victims Register may contact you for further information.

If you are not able to provide a copy of the intervention order, please provide as much information as possible about the intervention order in the fields below.

The Youth Justice Victims Register may need further information from you that will help to support your application.

After you submit your application, your application will be reviewed and the Youth Justice Victims Register will contact you to discuss alternative documentation that you may be able to provide.

You are about to send your application to your nominee to complete their section. 

When you click Submit, an email will be sent to your nominee.

Your nominee will need to: 

  • confirm their details 

  • upload their identification documents 

  • sign their declaration 

Before you submit, please let your nominee know to expect an email from the Youth Justice Victims Register. Ask them to check their spam or junk mail folder if they do not receive it. 

IMPORTANT

After your nominee completes their section, you will receive an email notification. You must then complete your own declaration before your application can be finalised and submitted to the Youth Justice Victims Register for assessment. It may help to ask your nominee to let you know once they have submitted their section.
